A tempting answer is an aggressive interior attack through the front door on the reasoning that no visible flames means a small fire. The better decision treats the front door as a potential air track into an oxygen-starved basement: keep it controlled, favor ventilation located high and away from crews and the attack path, and place a hose line before any opening is enlarged. A tempting choice is committing the entire first crew to the occupant's door through smoke. A better decision splits tasks by exposure: the closed door is a barrier that likely buys time, so one crew can protect that path while another cuts off fire spread from the kitchen toward the stairs. The error to avoid is treating life safety as one undifferentiated task instead of a ranking of separate exposures with separate timelines.

The question asks what you write and what you do. A tempting answer omits the failure because the nozzle ultimately performed. The reason matters: a pattern hidden behind clean logs is precisely what prevents a future crew from learning that an item has a history, and the record exists for that future crew, not for this shift's convenience.
